(formerly ‘Partnerships for Older People Projects', POPP)

Upstream is a key model for Devon's successful bid to the Department of Health in the second round of the POPP programme, worth £3.8 million to Devon over two years (2007/09) to Devon for the reorganisation of health and social care services for older people.  The community mentoring approach, the development of Healthy Living Communities in the voluntary sector, and the integration of the voluntary sector into GP multi-disciplinary teams are Upstream initiatives that will be disseminated through Devon under the POPP programme. This will fulfil Upstream's original aim to ensure that the lessons learned from the project would become an essential part of redesigned statutory and voluntary services. Upstream has won a consortium bid to deliver ‘community mentoring' throughout Devon (excluding Exeter).
