
Complex Care Teams (CCT) for GP surgery clusters were introduced as part of the Partnerships for Older People’s reorganisation of health and care services in Devon. The Teams included a cross-section of professionals as well as a Voluntary Sector Representative (VSR) to represent the views of the voluntary and community organisations involved in supporting the delivery of health and social care in the area. Upstream provides the VSR for the CCT in the Crediton area of Mid Devon.
The VSR attends weekly multi-disciplinary CCT meetings and receives referrals from CCT core members and primary care health workers, communicating closely with the CCT Co-ordinator based at the Crediton Hospital. The VSR raises awareness of the range of voluntary and community sector opportunities and resources available to support people in the community, and may also report unmet needs to identify services required in order to build community capacity in the future.
